For people whose admission date has not yet been scheduled, the hospital will make contact at a later date.
Procedures on the day of admission (first floor Admissions Reception) from 8:45 a.m.

What you need for the admission procedures
Patient registration card, health insurance card and medical certificate,
seal stamp, admission application form, eligibility certificate for ceiling amount application (for those applying for high cost medial treatment)
Should any details on your health insurance card change during your inpatient stay, or if you lose eligibility, ensure you inform the Admissions Reception on the first floor of the matter.
If you wish to change the day or time of your admission, or cancel it altogether, contact the Admissions Reception in advance by phone.

What to bring when admitted
Toothbrush, toothpaste, body soap, shampoo, conditioner, razor etc.
Only chopsticks, spoon, teacup etc. are provided with meals. If you wish to use any of your own items, please ensure you bring them.
Pajamas, underwear, gown or cover-up
Comfortable, non-slip indoor shoes, bath towel, tissue paper, container for dentures, diapers if necessary, disposable wipes, body wipes etc.
Bring all the current medicines you are taking together with the information sheets
The bringing in of blades such as scissors, razors, fruit knives and nail clippers into the hospital is not permitted. Your cooperation in this regard is appreciated.

Hospital Life
①During your hospital stay, please follow the directions of the doctors and nurses, and apply yourself to your recuperation. Please consult with your doctor or nurse about bathing, walking, visiting other medical departments, etc.
②If you want to go out or stay out overnight, please obtain permission from your doctor first, then fill in the required notification form, and submit it to your nurse. If you are confirmed to have been absent from your hospital room for a long time due to an unauthorized overnight outing, etc., we may contact your family or request a search from the police.
③To prevent theft, patients and visitors are requested not to bring large sum of cash or precious metals (rings, necklaces, etc.). Please be sure to keep any valuables you have in the built-in safe. Our hospital is not responsible for theft, etc. that occur under the patient’s control.
④For safety reasons, the ward entrances and the Day Room are kept locked from 22:00 to 6:00 the following morning.
If you want to enter or leave during this time duration, please ask your nurse.
⑤Using open flames is strictly prohibited to prevent fires.
⑥Smoking and drinking alcohol are strictly prohibited during your hospital stay, including your attendant family. Electronic cigarettes are also prohibited.
If you or your family smoke or drink alcohol, you may be discharged from the hospital.
※Smoking is prohibited anywhere on the hospital premises and the surrounding roads (including sidewalks).
⑦If you damage any property of the hospital, you may be asked to pay at cost. If you use abusive language or engage in violent behavior towards other patients or hospital staff, you may be discharged from the hospital or we may call the police.
⑧Please do not move out of the premises with an infusion pump or other instrument attached to your body, otherwise, the instrument may break down due to vibrations caused when you are moving. In addition, if the instrument fails in normal function due to a breakdown or dead battery, it may interfere with treatment.
⑨Please remove any nail polish, pedicure, etc. before admission. Please trim your nails before admission as we do not lend out nail clippers in principle.
Requests for Safety
・You will be asked to wear a name band (printed with your name, etc.) on your wrist or ankle.
・When you receive medical procedures, such as examinations or injections, you will be asked to give us your name. Please confirm this together with our staff.
○To continue treatment properly
・Depending on the medication you are taking now, you may be unable to receive an endoscopic examination or surgery. Therefore, please inform your doctor about the medications you are taking now, including one-shot medicines, before you are admitted to the hospital.
・Please inform your ward pharmacist about all the medications you are taking now, including internal medications and external medications (adhesive skin patches, ointments, eye-drops).
・At the time of admission, be sure to check with your doctor, pharmacist or nurse whether you may continue the medications you brought with you.
・Please properly inform your doctor or nurse of your physical condition. If there is any change in your physical condition, or if you have a pain or leakage from IV drips, please inform your doctor or nurse immediately.
・Unless you have been referred by our hospital, you are unable to visit any other medical institution during your hospital stay. If you run out of medication prescribed by another hospital before your hospitalization, for instance, please consult with your doctor, pharmacist, or nurse.
○To prevent tripping and falling
・During hospitalization, your physical strength and motor functions may decline due to illness, injury, and the physical effects of treatment. Even if you think you can walk on your own, your body may not move as intended, and you may trip at an unexpected time, leading to an accident, such as a fracture or bruise. To prevent tripping, our hospital recommends to wear closed-heel shoes. Please prepare them beforehand.
Please keep the area around the bed neat and well-organized, and do not put anything on the bedside table.
・We may consult with your family about ways to secure your safety. We appreciate your understanding and cooperation.
○To prevent infection
・To protect patients from infection as well as to prevent the spread of infection to others, our staff may wear gloves, masks, gowns, etc.
・We may also ask your visitors and your family to wear masks and gowns. We also recommend them to wash their hands and gargle upon arriving at the hospital and returning home.
・If your visitors and your family are ill with a fever, cough, etc., please tell them to refrain from visiting.
・Please use hand sanitizers provided by the hospital during your hospital stay.
・Mask vending machines are on the 1st and 3rd floors.

Restrictions on visits to hospital wards
Effective Tuesday, April 1, 2025, we will relax the restrictions on visiting the wards.

After filling out the “Ward Visit Form,” you will be given an acceptance sticker, which you are required to wear.
